With Bruce Jenner revealing his new female identity, Caitlyn, in a Vanity Fair photo spread this week, gender reassignment is in the news.
It’s a subject we haven’t spoken about often, though we’ve long offered transgender breast surgery at our clinics. So in the light of Caitlyn’s going public, we thought it was a great opportunity to discuss this often overlooked area of plastic surgery.
Gender reassignment surgery (GRS) – sometimes known as ‘gender confirming surgery’ – is a procedure (or set of procedures) designed to make transgender patients’ bodies better match up with their internal identities.
Although patients choose to undergo gender reassignment for a variety of reasons, most seek surgery to combat strong feelings of physical dysphoria, social dysphoria, or both.
Physical dysphoria is a strong feeling of unease at the mismatch between personal identity and body. We all have an internal monologue – the constant voice that guides us – and if this belongs to a gender that isn’t reflected when we look in the mirror, it can simply feel wrong.
On the other hand, a person suffering from social dysphoria feels ill at ease with the way others relate to them. Being treated like ‘one of the lads’ when inside you feel anything but can be deeply upsetting – a constant reminder that you are not the person you want to be.
Either or both of these states of mind can drive people to seek gender reassignment. And this can mean different things to different people. Some feel they need genital surgery in order to feel complete in their new identity.
Some seek facial surgery so their features better match their new physique. But for many, a breast enlargement is all they need to feel happy in their skin.
Reasons for seeking transgender breast surgery
Transitioning male to female patients will usually experience some breast development from their long-term hormone therapy. In most cases, this will result in A-cup size breasts, although occasionally they can grow larger.
Breast enlargement is generally performed when hormone therapy fails to result in satisfactory enlargement of the breasts. Transgender breast surgery gives patients rounder, fuller breasts with better projection from the chest.
The procedure can help transgender women achieve a more feminine physique, and enhance life in several ways. Breasts can make a new woman feel more secure in their new identity, more sexually attractive, more in proportion, and better able to fit into the clothes of her choice.
Does transgender breast surgery differ from regular breast surgery?
The answer is no, not really. Genetic male breast tissue is really not much different from female breast tissue, so transgender breast surgery uses conventional breast enlargement techniques.
Just like any breast enlargement, implants are inserted underneath the existing breast tissue through incisions in the creases under each breast. It is usually performed on an outpatient basis under general anaesthetic and takes around 1-2 hours to complete.
